- Abstract
- 목적: 유방암 수술을 받은 환자의 국소 지역성 재발의 진단에 있어서 fluorodeoxyglucose positron emission tomography (이하 FDG-PET)/CT에서 새롭게 발견된 과대사 병변의 임상적 의의와 이에 대한 2차 검사의 유용성을 알아보고자 한다.
대상과 방법: 2008년 1월부터 2011년 7월까지 수술적 치료를 받은 922명의 유방암 환자 중 정기 추적검사에서 특별한 이상이 없었지만 FDG-PET/CT상 국소 지역에 새롭게 과대사 병변이 발견된 20명의 영상을 분석하였다. 모든 환자는 과대사 병변에 대한 2차 검사(초음파 = 14, 흉부 전산화단층촬영 = 6)를 시행하였다. 17명은 조직학적으로, 3명은 영상의학적 추적관찰을 통해 진단되었다.
결과: 과대사 병변이 발견된 위치는 액와부 림프절(n = 7), 유방실질(n = 6), 흉벽(n = 3), 경부 림프절(n = 3), 속가슴 림프절(n = 1)이었다. 새롭게 감지된 과대사 병변 20예 중 10예가 조직검사상 악성으로 진단되었다. 초음파에서 7예는 악성이 의심되었고 이 중 5예가 조직학적으로 악성이었다. 흉부 전산화단층촬영을 실시한 6명의 경우 5예가 악성이 의심되었으며 모두 조직학적으로 악성이었다. 2차 검사에서 악성이 의심된 예의 83.3%(10/12)가 조직검사상 악성이었다.
결론: 유방암 환자의 정기 추적검사 중 FDG-PET/CT에서 새롭게 발견된 과대사 병변은 국소재발의 가능성이 높으며 따라서 이에 대한 적극적인 2차 검사와 필요한 경우에 추가적인 조직검사를 시행하여야 한다.
