- Abstract
- 목적: Four dimensional T1-weighted high resolution imaging with volume excitation keyhole technique (4D THRIVE keyhole technique)을 이용한 동맥기 영상에서의 국소 간 병변의 발견율 및 조영증강 양상 분석을 통한 임상에서의 응용 가능성과 국소 간 병변의 특성화에 대한 유용성을 평가하고자 하였다.
대상과 방법: 총 115개의 국소 간 병변이 있는 38명의 환자를 대상으로 4D THRIVE keyhole technique 영상이 포함된 자기공명영상을 시행하였다. 환자가 한 번 숨을 참는 동안 삼중시기 동맥기 영상을 획득하였고, 2명의 복부 영상 전문의가 병변의 발견율, 영상의 질, 병변의 명료성, 조영증강 양상을 각각 평가하였다. 영상의 질은 5점 척도를 이용했고, 병변의 명료성은 병변과 정상 간 실질의 대조도가 가장 높은 영상을 선택하였다. 각 병변의 조영증강 양상에 대해 분석하였다.
결과: Phase III에서 병변 발견율이 가장 높았다. 영상의 질은 3점 이상이었으며, 일치도는 fair였다. 병변의 명료성은 phase III의 동맥기 영상이 가장 높았다. 간세포암(n = 38)은 다양한 조영증강 양상을 보였고, 전이암(n = 14)은 환형(n = 6), 균질함(n = 3), 조영증강 없음(n = 5)을 보였으며, 대부분의 혈관종은 균질성(n = 6) 조영증강 양상을 보였다.
결론: 4D THRIVE keyhole technique을 이용한 간의 삼중시기 동맥기 자기공명영상은 비교적 낮은 발견율을 보였으나, 영상의 질은 임상 응용 가능성이 있으며, 국소 간 병변의 진단 양상의 특성화에 도움을 줄 수 있다.
