| dc.description.abstract | 주상골 골절은 흔한 수근골 골절로 진단을 정확히 하고 적절히 치료하면 대부분 쉽게골유합을 얻을 수 있다. 비전위성 골절은 보존적 치료로도 유합을 얻을 수 있으나 젊고 활동적인 환자는 보존적 치료보다 경피적 나사못 고정을 통한 견고한 고정으로 조기에 유합을 얻고 정상적인 생활에 복귀시키는 것이 좋을 것이다. 주상골 골절의 불유합은 대부분 고식적인 골이식과 견고한 나사못 고정술로 유합을 얻을 수 있다. 고식적 골이식이 실패한 경우나 근위부 골편의 무혈성 괴사가 동반되어 유합이 어려울때는 유경 혈관화 골이식이 유용하다. 불유합부에 과도한 곱사등 변형이나 단축 변형등이 근위부 골편의 무혈성 괴사와 동반되어 있는 경우는 대퇴골 내과를 공여부로 하는 유리 혈관화 골이식이 마지막 대안이 될 수 있다. | - |
