- Abstract
- 목 적: 외측 및 후내측 두 개의 도달법을 이용하여 치료한 Shatzker 제6형 경골 고평부 골절의 치료 결과를 분석하여 방사선학적 및 임상적 결과를 알아보고자 하였다. 대상 및 방법: Shatzker 제6형 경골 고평부 골절에 대하여 외측 및 후내측 도달법을 이용하여 치료받은 후 평균 16개월간 추시하였던 11명 12예를 대상으로 방사선학적인 분석을 통하여 유합 기간 및 선열, 정복의 소실 정도를 평가하였고, 임상적인 결과는 슬관절의 운동 범위, Knee Society Score, UCLA activity scale을 분석하여 평가하였다. 결 과: 전 예에서 추가적인 수술 없이 골유합을 얻었으며, 골유합에는 평균 15주가 소요되었고 정복의 소실은 관찰되지 않았다. 최종 추시 시 슬관절의 운동 범위는 평균 132도였으며, Knee Society Score는 평균 85점이었고, UCLA activity scale은 술 전 평균 9.6점에서 술 후 평균 5.7점으로 감소하였다. 결 론: 두 개의 도달법을 이용한 Shatzker 제6형 경골 고평부 골절의 치료는 견고한 고정에 의한 조기 관절 운동을 통해 우수한 관절 운동의 회복과 비교적 양호한 슬관절 점수를 보여 권장할 만한 수술 방법의 하나로 생각한다. 그러나 고에너지 손상으로 환자들의 운동능력 (sports activity)에는 많은 저하를 초래하였다.
