Castleman 병을 동반한 홍색피부건선 1례

Castleman disease (CD) is a heterogeneous group of polyclonal lymphoproliferative disorders with overlapping clinicopathological features. It is classified into unicentric CD (UCD) and multicentric CD (MCD), with MCD further subdivided into idiopathic MCD (iMCD) and other etiologies. There have been several reports of CD associated with psoriasis. We report a case of a 74-yearold female with long-standing psoriasis who was eventually diagnosed with CD. After discontinuing psoriasis treatment for more than one year, she presented with a generalized erythematous, scaly rash covering her entire body, along with recent weight loss and fatigue. Based on a clinical diagnosis of erythroderma or exfoliative dermatitis, treatment was restarted with acitretin and topical corticosteroids. Laboratory findings revealed marked anemia, increased myelocytes in peripheral blood, and hypoalbuminemia. She was referred to the hematology department for further evaluation and was diagnosed with iMCD following hospital admission.
